Treatment for Diffuse Unpatterned Alopecia?

I have Diffuse Unpatterned Alopecia. Since I was 20, i started hair loss. Now i am 28 and its serious. I want to get hair transplant surgery, with you but when I was doing some research, i think I don’t have enough donor hair for treatment. My hair is very thin and balding. It does not have patterns. Its just thin over all. Only parts that are thick are the very front on each side (not side of the head but top front side) and maybe some at the back. But compared to others in your result photos, i have no donor hair. Is there any treatment for someone like me? Please help.

Treatment for Diffuse Unpatterned Alopecia (DUPA), especially when considering hair restoration, can be difficult due to one reason… the law of supply vs. demand.

This means we have to look at the amount of area affected by male pattern hair loss and the amount of hair that we have to work with in order to give a cosmetically pleasing result.

This doesn’t necessarily mean that we must have a high density, large coverage Rahal Mega Session™ with 7000 grafts in one procedure. This means achieving moderate density and designing a hairline that frames the face. This alone can and does go a long way in achieving an improvement so you can look and feel younger.

We can assume that we can rule out a Follicular Unit Transplant (FUT) to address your needs because the linear scar would be more easily visible with even longer hair lengths. Typically FUT scarring can be hidden with moderately short hair cuts but this is assuming that an average level of density exists in the donor region.

Lower than average donor density, as in your case, leaves less room for hiding linear scars, thus Rahal FUE™ is indicated because of the lack of a linear scar altogether. However, the challenge may be that we are dealing with DUPA, which means that we cannot reliably count on the donor hair to last indefinitely as we generally would with donor hair from a typical non DUPA donor zone.

However, depending on the overall size of your current hair loss on top of your scalp and if you are agreeable to taking preventive medications such as Propecia and Rogaine then we may be able to find a suitable number of grafts to take from the area of your donor zone that you indicated as being “thicker”.

Hair Transplant without Taking Propecia?

I am a 47-year-old male. I have been taking Propecia and have been applying Minoxidil 1-2 times per day for the last 4 years. Not consistently but pretty regularly. Despite my efforts, I am still losing my hair on the mid scalp and the lateral hump as well as the crown. My hair loss is starting to show!! I actually feel the itching on a regular basis and what a horrible feeling when sitting in a meeting and thinking while listening to someone talk to you that I am losing my hair. My question is: if I have a transplant, will my transplanted hairs eventually follow the same path of my other hair? Can I get a hair transplant without Propecia?

Hair transplants replace hair. Not prevent hair loss. Only FDA-Approved medications like Propecia can stop hair loss.

I am sorry for your problem. I know hair loss is frustrating not to mention greatly concerning.

The first thing I have to ask you is… are you taking generic finasteride or name brand Proscar or Propecia?

I ask because we have seen, as have other clinics, an inconsistency with the quality of finasteride that is not from Merck Pharmaceutical. They are the original manufacturers of finasteride.

I always recommend that patients take name brand Proscar because when the tablet is quartered it winds up being very affordable and the good thing is that you know you’re getting the real medication.

The itching you feel is potentially a reaction to inflammation in your scalp. There is a correlation between scalp inflammation and hair loss with some hair loss sufferers actually reporting that their scalp “hurts” at the base of each hair. For this I recommend you start with an anti-inflammatory agent such as Nizoral shampoo. This will help to reduce inflammation and the itching associated with it.

Can you get a hair transplant without Propecia?

Yes…

You can get a hair transplant without taking Propecia. The transplanted hairs should continue to grow for a lifetime if taken from the safe donor zone.

The issue, however, is not the transplanted hair but rather your native hair…

How Much Does Hair Transplantation Cost?

I need a hair transplant. How much does hair transplantation cost for 4000 grafts?

How did you come up with the estimate of 4000 grafts?

Is this through self diagnosis after reviewing online galleries and reading discussion forums? Or did you receive an estimate from another clinic and want to know what we charge for the same number of grafts?

If self diagnosed, then there is a strong possibility that your self assessment is incorrect due to vastly different hair characteristics. Many times one will look at results online and not take into account the hair texture (fine vs. coarse). Is the hair wavy or straight? Even hair color can and will influence the number of follicular units necessary to achieve a cosmetically acceptable result.

If you received an estimate from another clinic, then this too can be incorrect, as many times two seemingly reputable clinics can give estimates that are very different from each other. One clinic may specialize in coverage while the other clinic may specialize in density.

We then must look at expectations…

Sometimes patients will overestimate their own needs because they feel that if they transplant a lot of hair into their existing hair, even if it is not visibly thinning, that this will somehow prevent future loss from being noticeable because so much new hair has been put in place “proactively”.

On the surface this seems like a viable approach, however, when one considers this further they realize that this is a counter productive philosophy. A friend of mine refers to this as the “glass of water” analogy. If you take a full glass of water and leave it outside for a while you will notice that the amount of water will be less due to evaporation.

The evaporation represents hair loss.

You can’t fill the glass with extra water to stave off the issue of evaporation because there is no more room for more water.

It’s the same thing with hair restoration…

You can’t add more hair to an area that is not experiencing loss because there is no room.

I know of cases where a doctor tried to to do this but it only lead to permanent shock loss that actually made the patient’s scalp look thinner than before the procedure.

How Long Does Propecia Take to Work?

Just picked up a prescription of Propecia and will start taking it tonight. How long does Propecia take to work? And when can I expect to see results? I’m not really expecting any growth. Just want keep what I have and save the crown. I’m 26 and will get a hair transplant for the hairline with you in the near future.

For anyone not familiar with Propecia. It’s an FDA-approved pill that blocks the formation of DHT, the hormone responsible for male pattern hair loss.

While some men do get limited regrowth, mainly in the crown, Propecia is most effective at “maintaining the hair that you have”.

So the sooner you start taking it the better.

It’s a commitment though…

Propecia will only work if you continue taking it.

The Great ACell and PRP Injection for Hair Loss Hoax

After surgery, do you use any kind of ACell and PRP injection to aid healing and help retain native? I am considering a hair transplant, but I’m concerned that the hair on my crown – although it’s thin, there’s still about enough there – will be shocked into miniaturized state.

Platelet-Rich Plasma (PRP) is prepared by centrifugation. Some claim that PRP can promote new hair growth and enhance healing after hair transplantation. There is little evidence to support these claims.

PRP stands for “Platelet-Rich Plasma”. The process involves extracting the patient’s blood and placing the vile in a centrifuge. By spinning the blood at high velocities, core components are separated… one of which is platelet rich plasma.

The truth about PRP injections for hair loss…

Blood plasma contains growth factors that are released through the introduction of “thrombin” and “calcium chloride”. Some claim that when injected into the scalp, PRP therapy can promote new hair growth and enhance healing after hair transplantation.

I have yet to see evidence that supports these claims.

Acell hair transplant claims debunked…

ACell’s Extracellular Matrix (ECM), in the context of hair restoration, can apparently do amazing things such as… “eliminates scarring” and “regenerates transected hairs”.

Some clinics even combine PRP and ECM as an “add-on treatment” to hair restoration with the promise – or strong suggestion – that the patient will get better results.

Unfortunately, there is very little evidence that ECM actually works… other than adding to the bottom line of these clinics.

Understanding shock loss after a hair transplant…

There are two types of shock loss, temporary & permanent.

Temporary shock loss is thought to occur when blood flow is interrupted to the scalp. Typically when tiny incisions are made in the recipient area. Not to worry. These “shocked hairs” typically return at 3-5 months.

Permanent shock loss  can be avoided by shaving the recipient zone, so we can see the natural angle and direction of native hairs.

By making incisions parallel to native hairs, we avoid the primary cause of permanent shock loss… transection.

Permanent shock loss can also occur when miniaturized hairs shed. Don’t worry. Losing these miniaturized hairs will not affect the final result… they were on their way out within six months anyway.

Neither PRP nor ECM will prevent shock loss.

In your case, it sounds like “temporary shock loss” is a possibility, though I’ll need to view photos of your crown to determine the level of miniaturization.

How to Fix My Uneven Hairline? And Estimated Cost?

I had a hair transplant of 2300 grafts (at another clinic) by strip method 9 months ago. I’ve been applying Topical Finasteride and Rogaine mixture once per day. I also Apply RA plus Rogaine 2x per week as protocol from my surgeon. I am not happy with my hairline – it’s uneven – and am blown away by your hairlines! I have a posted a picture for your review. I would like to fill in the temporal recession areas more to create less of hard V Shaped look. Can you give me an estimate of the number of grafts you think it would take to soften the hairline? And how to fix my uneven hairline? Maybe lower just a fraction if you think it needs it. And also an estimated cost? I really appreciate your time. I look forward to your thoughts on my hairline. Bryan had a hair transplant at another clinic. The right temple is uneven and far more recessed than the right temple. The middle of the hairline is slightly off center as well. Dr. Rahal reveals the solution.

Sadly, your situation is far too common.

Natural hairline design takes years of training… and more clinics seem to get it wrong than right.

I estimate that 20% of my practice is devoted to “hairline repair”… and fixing the hairline design of other clinics.

I always suggest researching your hair transplant surgeon… are they recommended by the American Hair Loss Association? Do they specialize in hairline design? Maximum density?

These are just some of the questions every patient should be asking.

It sounds like you have a good handle on hair loss prevention.

Great job Bryan… Both Rogaine and Finasteride should help you take control of your hair loss.

Now on to your hairline…

From the one photo you sent, I can see that there are some inconsistencies with the placement of your hairline.

The first thing I notice is that the temple recession is too asymmetrical. Your right temple is far more recessed than your left temple.

It also appears that the middle of your hairline is slightly off center as well.

There is nothing wrong with asymmetry… it’s expected in nature. It’s important that the hairline is “balanced” though. You can achieve this with temple point and template angle restoration.

Will I Still Need to Use Toppik after a Hair Transplant?

Hi Dr. Rahal. I have a question about concealers like Toppik and Dermatch. Can I stop using them after a hair transplant procedure? Would the density be enough for me to stop being so depended on concealers?

Most Rahal patients do not use Toppik after hair transplant surgery. A Rahal Hairline™ at maximum fullness and density allows for unlimited styling options like… long, short, messy.

This is a very good question… but the answer is not a simple one.

Concealers such as Toppik and Dermatch have one job… conceal areas of thinning.

They do this by eliminating the visible contrast between hair and scalp, which effectively gives the illusion of a fuller, thicker head of hair.

The one problem with concealers is that they do not allow for a natural amount of scalp to be visible.

Even people with the thickest heads of hair will have at least some portion of their scalp visible… depending on the hair style, the length, etc.

For instance in the part line or the frontal hairline where there are natural undulations. Even the central part of the crown where the whorl pattern originates will allow for a small amount of scalp to be visible.

Concealers are used everyday in Hollywood…

I spoke to one of my celebrity patients about what’s required of him in the makeup department. He has a full head of hair and just wanted to lower his hairline to maintain a youthful appearance.

He told me that even though he has a full head of hair, he would have to wear concealers because… “Hollywood doesn’t like scalp.”

I was surprised by what he told me… but since he mentioned it I can’t help notice that I rarely see scalp on actors when watching a movie or television show.

So, back to your question… “Will i still need to use Toppik after a hair transplant?”

That’s entirely up to you.

I know this may sound confusing… but it has to do with what YOU feel is cosmetically acceptable.

Does FUE into Scar Tissue Work?

Hi Dr. Rahal, Hope all is well with you. I have a few questions about FUE into scar tissue.

  1. Do you perform many FUE procedures into scar tissue?
  2. Will FUE scars leave white or pink spots, or the donor area be detectable with a very short haircut (1/8 or 1/4 of inch)?
  3. What FUE graph failure percentage is the norm?
  4. Do you use Acell and manual punch for the FUE extraction (punch size, 0.8mm?)
  5. What can be done to increase the FUE density into scar tissue?

I haven’t had a hair transplant and have a good donor area . The scar is from an operation on both sides of the head, about 2 1/2 – inch long each (not too bad, but will show with very short hair). I live in the Washington, DC area.

Thank you for writing. I’ll address each of your questions separately.

1. Yes, we perform FUE into scar tissue at Rahal. Concealing scarring with Rahal FUE™ is a simple procedure and takes only 30-60 minutes.

Here is an example of a Rahal patient after FUE into scar tissue… before, immediately post-op and 9 months post-op. The scar is concealed… and the patient is excited that he can wear his hair short again.

2. There are two factors that determine whether or not the Rahal FUE™ extraction points will be cosmetically noticeable. First is the punch size. We routinely use a .8mm punch, as we find this can adequately handle most grafts with ease. Once the extraction points are completely healed, it’s exceedingly difficult to see any evidence of extraction in the form of obvious scarring or discoloration.

Pink dots as you referenced would indicate ongoing healing… and then white dots are when healing has finished and the fibrous scar tissue has settled. With the .8mm punch we find that the final circumference of the extraction point is cosmetically insignificant.

The second factor affecting the cosmetic appearance of the donor zone is the overall pattern of extraction. With the extraction pattern spread out strategically there is no perceptible reduction of donor density, which can be a real issue if not properly executed.

Too many times I have seen patients with FUE from other clinics come to me with donor zones that are patchy because the clinic removed too many grafts in a specific region of the donor zone. This is counter productive to the whole point of FUE, which is to have cosmetically insignificant visual indicators that a procedure has been performed to begin with.

3. You asked about the survival percentages of our Rahal FUE™ (which is a good question). With any clinic performing FUE, there were challenges in the beginning. When I started working with FUE 9 years ago (2005) the survival rates were below that of my “Rahal Mega Session™” survival rates.

However, as time progressed, so did our experience… and today our Rahal FUE™ survival rates are in the high 95% to 98% range (which equals typical FUT survival rates).

At this point the debate about survival rates between FUT and FUT is a moot issue.

The key to getting to this point was not just learning how to use the punch properly. Using FUE punch is a fairly straight forward affair. The challenge was learning how to utilize a punch with the various types of hair and even scalp tissue that presented themselves in our patients.

This is the learning curve that must be overcome before one can consider themselves proficient with Rahal FUE™ and is what allows us to have such high Rahal FUE™ survival rates today.

4. Acell is a product that was very popular online a few years ago but the excitement has someone diminished of late. I am still asked about it from time to time but to be honest , I’m not a fan of Acell. This is mainly because of how it is being marketed by some clinics in the United States with at least one clinic I know of actually stating outright that by applying Acell to Rahal FUE™ donor extraction sites they are seeing in some cases up to 80% regeneration.

To date there is no photo or video documentation to back up these claims.

There may be merit to the use of Acell for basic acceleration of healing post-op… but I find that with the .8mm punch used in our practice there is little use for the ambiguous improvements that may be had through the use of Acell because the donor sites already heal quite well with no external influence.

5. Increasing density into scar tissue via Rahal FUE™ is a fairly simple affair. Many clinics feel that Rahal FUE™ into scar tissue, because of it’s overall lower vascularity compared to healthy skin tissue, cannot support anything more than moderate density at best.

I feel that taking a blanket approach to this issue is shortsighted, as higher densities are indeed possible for some scar formations.

With a proper and strategically safe extraction pattern and a proper sized punch having your hair at ¼ or even 1/8th of an inch in length should allow for a cosmetically acceptable result in the donor zone… but to get a better handle on your case and for you to get a more detailed assessment I recommend you schedule your 100% free consultation today.

1) With FUE, are you able to transplant the 4000 FUE in one day or is it limited to a lower amount?
2) Can I get FUE without shaving my head? Or can I simply shave the back of the head?

Here is a before and after example of “Rahal FUE™” — the least invasive and fastest-healing hair transplant available. At Rahal we can safely harvest between 2000 to 3000 grafts a day (4000 FUE over two days).

Over the years we have become very proficient with Follicular unit extraction (FUE) extractions… and can safely harvest 4000 FUE over two days (between 2000 to 3000 grafts a day).

As with any hair restoration procedure though, harvesting 4000 FUE is dependent on a few factors, as every patient will present varying hair characteristics that can (and do) influence the rate of the extractions and time needed to perform the procedure correctly.

If hair shafts are wavy or curly, for example, this can slow the process, since extra care must be taken to avoid damaging the grafts during extraction. Scalp tissue alone can vary by patient… from firm and consistent that’s easy to work with… to more pliable tissue that’s soft and more difficult to work with.

We find that for a larger FUE hair transplant (4000 grafts plus) it’s best to perform smaller sessions over two days to get the desired number of grafts and results. This allows everyone involved to be more comfortable… both technicians and patients alike.

Allowing more time to do the job right offers a level of confidence that everyone can benefit from.

FUE without shaving head?

Shaving the donor area is a requirement so we can determine where the grafts should be extracted and avoid creating areas that look thin because over harvesting. You want to avoid removing too many grafts in any given area, which can lead to visual clues and an overall inconsistency that is cosmetically undesirable.

We shave the recipient area for two reasons…

First, we want to avoid damaging the native hair. By shaving the recipient area the native hair stands at it’s true angle and direction. When hair is longer it’s more difficult to see the spaces that need to be filled… and at such a short length there is nothing obscuring my field of view when I am making incisions.

This is very important because when one cannot clearly see what they are doing, the risk of transection (cutting and destruction of native hair during recipient site creation) increases significantly.

This is the number one cause of permanent shock loss after a procedure and shaving the recipient zone is the most effective way to eliminate this problem. There are exceptions, however, in that we might not shave if the hair in the recipient zone is of very low density… but this can only be determined on a case by case basis.

How many grafts do I need results examples.

Creating the illusion of a full head of hair with very few grafts is the “art and science of hair restoration”. Here are Rahal Hairline™ examples at 60-70% original density.

The amount of fullness (or density) that you can achieve depends on a few things…

We first need to consider the amount of donor hair that you have, in relation to the amount of loss you have experienced to date… or may experience in the future.

What i mean is that you only have a limited supply of grafts available to transplant in your lifetime… so we need to use them wisely.

That’s the “art and science of hair restoration”… creating the illusion of a fuller, thicker head of hair with very few grafts.

It’s really about creating more with less.

Have you heard of the 50% rule?

It states that when you first noticed your hair thinning under “harsh lighting” you had lost 50% of your original density. Say for example your original density is 80 grafts cm/2…. that means you noticed hair loss under harsh lighting at around 40 grafts cm/2.

Under normal conditions you didn’t notice any hair loss.

The 50% rule gives me a baseline… I know that I need to transplant above 40 grafts cm/2 (roughly) to create the illusion of a fuller, thicker head of hair.

Though I find that 50-70 grafts cm/2 achieves maximum fullness & density… so you can style your hair as you wish — long, short, parted, or “messy”.

Your hair characteristics also play role…

With coarser hair, we can use fewer grafts in general to create the illusion of a full head of hair because the diameter of each hair takes up a fairly large space and provides strong coverage.

Finer hair requires more grafts to achieve density due to the smaller diameter of each hair.

It is the same as comparing Redwood trees to Poplar trees. You only need a few Redwood trees to fill a given area… where you need many more Poplar trees to fill an area the same size.

Even hair color can make a difference in how a result turns out…

The amount of contract between hair color and scalp color greatly influences the perceived amount of density that a result achieves. Lower contrast helps aid in the appearance of density… while higher contrast makes it more difficult to achieve an appearance of density.

Hair loss is a very natural part of the aging process… and is overwhelmingly dictated by your genetic history.

How Bald Will I Be?

Take a look at your relatives hair loss pattern to get an idea of where you might be headed. This includes male relatives on your mother’s side as well as your father’s side, since both sides can contribute equally.

Here is what to look for…

Are there multiple men in your family that have hair loss… and if so to what degree? Is there a common hair loss pattern?

If yes, this could indicate your future hair loss pattern.

Don’t panic though. Realize that it’s NOT an exact science… only a clue.

If it looks like hair loss is in your future, then consider hair loss prevention medications such as Rogaine and Propecia (for men only).

Both are FDA-Approved… and proven to prevent and reduce hair loss.

These medications are not cures though.… and will only work if you continue taking them. If you stop, you will likely lose any hair you have gained within a few months.

Arthritis Drug No “Miracle Cure” for Hair Loss

In June 2014, researchers at Yale University reported startling findings: By using tofacitinib citrate (commonly sold as Xeljanz and Jakvinus), a drug used to treat rheumatoid athritis, the Yale research time was able to regrow scalp hair and body hair in a patient with alopecia universalis.

This might make men experiencing hair loss ranging from a receding hairline to completely bald pause for hope…

Different Types of Hair Loss, Different Treatment Needs

Androgenic alopecia—male pattern baldness—and alopecia universalis are two very different conditions with one superficially similar symptom: Hair loss. The differences lie in the both the mechanism of hair loss, the extent of it, and how these conditions are treated.

Tofacitinib appears to spur hair regrowth in a patient with alopecia universalis by turning off the immune system attack on hair follicles that is prompted by the disease, the Yale team reported.

There are currently no drugs on the market that can completely halt or reverse male pattern baldness. Finasteride and Minoxidil, commonly sold as Propecia and Rogaine respectively, can help slow and manage hair loss, but hair transplant surgery is still the only permanent solution to restore your naturally-growing hair.

With male pattern baldness, genetics and hormones are the key factors in hair loss. Dihydrotestosterone, or DHT, is the primary driving mechanism in male pattern baldness. DHT is a potent form of testosterone. The hair follicles on the top of the scalp and at the crown are sensitive to DHT in men with male pattern baldness.

Theories as to the reasons for male pattern baldness, and why those hairs in particular are sensitive, abound. One theory suggests that male pattern baldness might have evolved as a defense against prostate cancer. High levels of DHT can enlarge the prostate and lead to prostate cancer.

The theory suggests that scalp hair began to fall out in the presence of DHT to expose more skin to direct sunlight. More sunlight means the body synthesizes more Vitamin D. People with low levels of Vitamin D are more likely than people with normal levels of Vitamin D to test positive for prostate cancer. Male pattern baldness, according to this theory, is the body’s way of defending itself.

Norwood Scale

The Norwood Scale illustrates the stages of androgenic alopecia, or male pattern baldness. It’s what most people think of when they think “bald”.

Alopecia universalis is a different story. Alopecia universalis is the most extreme form of alopecia areata (loss of hair in a single area of the body). Alopecia totalis, the loss of all hair on the head, is another form of alopecia areata.

All forms of alopecia areata, including alopecia universalis, are believed to be autoimmune disorders. In the case of alopecia universalis, the body’s immune system attacks hair follicles the way it would attack a virus or an infection, causing the hair to fall out.

A Rare Treatment for a Rare Disease

Alopecia areata affects only about 0.1% of the population, or 1 in 1000. Alopecia universalis, its most extreme form, affects far fewer.

Male pattern baldness, on the other hand, affects many men, if not the majority of men. A 1998 study suggested that as many as 42% of all men, or nearly 1 out of 2, will have Norwood Stage 3 male pattern baldness or greater. That doesn’t include Norwood Stage 2 and 2A.

Propecia Not Working To Stop His Hair loss? Empty Stem Cell Baldness “Cures”? sirius67 Had Enough.

“I bought some finasteride online… unfortunately I got some sugar pills and I started to lose a lot of my hair”

Before hair restoration, sirius67 did what many hair loss sufferers do: He held out for a miracle cure, this time promised in the form of a supposed stem cell cure for baldness: “recoverup hair regeneration,” sirius67 wrote, “what do you guys think about this? Is it for real?”

He tried to navigate the waters of online pharmacies looking for a reputable seller of generic finasteride, or Propecia: “I bought finasteride [online] and the first order was cipla brand, very good stuff, the second one I ordered from the same site… unfortunately I got some sugar pills and of course after only a few weeks I started to lose a lot of my hair.”

Around 2011, he’d had enough. With Propecia not working to stop his hair loss, it was no longer good enough to slow down it down or cross his fingers and hope for deliverance. If he was going to take control, he needed to take control immediately.

We spoke with “James”, a Rahal FUE Hair Transplant patient from Tampa, FL. He told us his hair loss story. It was simple enough: He had a great head of hair. He lost it. And he got it back again… with Dr. Rahal’s help.

I had great hair when I was younger. I was actually voted ‘best hair’ in my high school class! But by senior year, I began to see I was losing my hair and it got bad pretty quickly. Through my college years, my hairline receded quickly.

I was thinning at the back of my head and had a really bad receding hairline. Overall, my hair was kind of thin. When I graduated, in my early working years, I lost more and more hair and it was pretty difficult for me to take.

Being voted ‘best hair’ then going back balding would have been very embarrassing. Now I can go with full confidence.

I had my hair transplant operation two years ago when I was 29. I have better hair now than when I was in high school! My hairline and my temple area are great. The hairline is very, very important to me because it’s something I see every day. I wanted a hairline that looked natural at the temples. Now I use products in my hair, or comb my hair back, or to the side, no problem.

Painful Memories

Like many men, ‘James’ decided to hide his hair loss any way he could. Hats. Combovers. No control.

I remember when I used to wear hats all the time. If I was in the wind without a hat, my hair would blow back and my hair loss would be obvious. I remember going on a date, going on a roller coaster where they had cameras taking pictures as you’re going down. I looked really, really bald. Now I’m outside in the wind, playing golf, basketball, tennis, all kinds of sports. I don’t have to wear hats. I’m very confident my hair looks good even in the wind.

I used to comb my hair to try to cover up balding as best I could, but when I’d go swimming and come up out of the water, I’d have this little strip of hair in the front of my head. Now I can go in the pool and have a full head of hair, not a tiny strip in the front. I live in Florida and the beach is a big deal. Now I can go swimming with full confidence.

Here’s another example. I went home this past Christmas and my family had pictures of me from a couple Christmases ago. In the picture, I’m balding and I looked 10 or 20 years older than I was at the time. I saw that picture and it brought back the feelings when I was at that point. Today, my whole family says, “We cannot believe your hair!” Wow, what a blessing to have it so different today.

Sure, looks are superficial. Physical appearance is not the most important thing, but for me, balding was a struggle just because I felt like I really was becoming less attractive and desirable than I once was. It felt like I was getting old, looking old, and it was difficult to handle emotionally.

I talked with Dr. Rahal about my goals and he gave me a realistic opinion of what he thought would be possible for me. It was better than what I was expecting. The surgery lasted quick a while. I got over 3000 grafts. It was a long surgery.

The surgery itself wasn’t really very painful at all. I had no real pain, they numbed me up. That night I had a pretty good headache but that night was the toughest part. After that, everything went well and the pain was nothing. I had no pain the next morning. I followed the instructions Dr. Rahal gave me very closely, as prescribed. I used a spray, an oil, and the vitamin E oil he prescribed.

Balding is a really difficult thing to go through. More so than anything else. As a man, when I was balding, I felt I was losing my sense of identity, my sense of worth, my sense of manliness. Did I feel less virile? Absolutely. It’s amazing how differently people perceive me, good or bad, with or without hair. I definitely see a difference in how people perceive me. I don’t know if that’s because I look different or because I have more confidence now. I don’t feel insecure. I feel a lot of it has to do with having a full head of hair. I see a definite difference, a real-world difference, with a full head of hair.

Dating is important to me and I’m dating around quite a bit these days. I’ve taken a lot of girls on dates and had a lot of girlfriends. I’m single but waiting for the right lady to come along. There’s one I’m interested in now but we’re just starting to hang out together and we’ll see where the good Lord leads. I’m trying to be patient. I met her after my surgery.

I’m 31 and I’m ready to meet the right lady and settle down. Before, I felt I might not be able to be attractive enough to attract the woman I really wanted, that I couldn’t get the best mate. I thought I’m not good looking enough because I’m bald. I wasn’t confident enough to talk to the pretty girls because I wasn’t confident in myself.

The girls, the dating, the relationships–it’s all great but the biggest thing is really being confident in yourself. That’s more important than anything. You look at yourself in the mirror and it’s a difficult thing to deal with when you’re not as attractive as you once were. It feels like there’s something wrong. Dating pretty girls is nice but that’s secondary to feeling confident in yourself. Everything follows when you’ve got a core of self-confidence. Life is better in general.
